Consult your plans Outline of Coverage or call your plans customer service to have them price this service for you using CPT code 88360-1.
If they refuse to provide you a price know that you are entitled to know the price before the procedure is done or at least know the baseline cost so that you can be a consumer of healthcare and not just a user. You wouldn't buy a car without knowing the price even if you have options to include add-ons you would know the base price. The same is true for your health care services and diagnostic testing. Know before you go. It's your money and your body!
